Waiver and Release of Liability

Plain English Summary

1. Assumption of Risk

I understand and acknowledge that participation in play activities at Fun & Focus Playhouse involves inherent risks, including but not limited to:

• Falls, bumps, and bruises common to active play
• Contact with other children or equipment
• Allergic reactions despite reasonable precautions
• Exposure to communicable illnesses
• Emotional distress from separation or social interactions

I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of Fun & Focus Playhouse or others.

2. Release and Waiver

In consideration for my child(ren) being permitted to participate in activities at Fun & Focus Playhouse, I hereby release, waive, discharge, and covenant not to sue Fun & Focus Playhouse, its owners, officers, employees, and agents from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in activities.

3. Indemnification

I agree to indemnify and hold harmless Fun & Focus Playhouse from any loss, liability, damage, or costs, including court costs and attorney fees, that may occur due to my child(ren)'s participation in activities, whether caused by my child's negligence or otherwise.

4. Medical Authorization

In the event of an emergency, I authorize Fun & Focus Playhouse staff to secure emergency medical treatment for my child(ren). I understand that I am responsible for all medical expenses incurred.

I have accurately disclosed all medical conditions, allergies, and special needs relevant to my child(ren)'s participation and safety.

5. Photo/Video Release

I grant Fun & Focus Playhouse permission to photograph and/or video record my child(ren) during activities for use in promotional materials, social media, and marketing purposes. I understand I may opt out of this by notifying staff in writing.

6. Parental Supervision Acknowledgment

I understand and agree that:

• I must remain on the premises at all times during my child(ren)'s play session
• While staff supervise play areas, I retain primary responsibility for my child(ren)
• I will respond promptly if staff require my assistance
• I will inform staff immediately of any concerns or incidents

7. Facility Rules Agreement

I agree to ensure my child(ren) follow all facility rules, including:

• Age-appropriate play area usage
• No running in designated areas
• Respectful treatment of other children and staff
• Proper use of equipment
• No food or drinks in play areas (except water)

8. Illness Policy Agreement

I agree not to bring my child(ren) to Fun & Focus Playhouse if they are experiencing fever, vomiting, diarrhea, unexplained rash, or other symptoms of contagious illness. I understand my child(ren) may be sent home if symptoms are observed.

9. Binding Agreement

This waiver shall be binding upon me, my heirs, executors, administrators, and assigns. I have read this waiver, fully understand its terms, and sign it freely and voluntarily.

This waiver is signed electronically when you complete registration and is valid for all visits unless revoked in writing.
